Transcription Procedures/Documents


The following core templates are created as a baseline as to what should be dictated in the reports. You are ultimately responsible for the final quality of your reports. You must be responsible for double-checking the validity and current standards used in these templates. We will regularly make changes to templates where the physician has requested changes and/or additions to their "must use" template. We want to help you be as productive as you can be, and as accurate as possible with the information we provide.

The signature block of ALL Alabama reports requires the physician's specialty to be included.


NOTE: Use the code on your paperwork to determine the correct location.

Physician MD/DO Specialty Board Status State Template

Request: Please share any templates you may have for physicians listed that do not currently have templates available. Ultimately, we all benefit from having these available to us, and as we all know, they can save us a tremendous amount of time. Our goal is to have a template for every physician, for all to use. Again, please use them wisely and be certain all critical, necessary information is correct before submitting final reports. Please email any templates you would like to submit to: bburton@mdsiphysicians.com. Also, please help us identify them by indicating if a physician has requested we use a specific template.

Physician Dictations



Follow these instructions:

  • Get a job assigned to you, either from MDSI or by clicking the Get Job button above.
  • Click the Job # below to download the dictation.
  • Open the dictation in your transcription software and transcribe.
  • After you're finished transcribing the document, save it with the filename Claimant-Name-Spec.doc (example: John-Doe-IM.doc)
  • Click the Upload button for that job and select your doc file to upload the document.
  • You may upload a file again if you need to make corrections.
